In Home Services (Family Builders)


Family Preservation (Intensive In-Home):

Therapists and/or Case Managers work with the family unit and are available for 24 hour crisis intervention. The intense interventions involve dealing with the immediate crisis, engaging the family, assessing the problem, and developing the goals.


Home Based Therapy and Education:

Therapists will provide services in the home and utilize a Family Systems approach which allows the flexibility for the Therapist to work with an individual and/or family unit. The focus will be communication, parenting skills, coping skills, behavior management and situational responses.


Family Therapy:

Therapists work with the family unit with the focus being on communication, relational, and coping skills. The family will learn to develop problem solving skills and build networks of support in order to help themselves be self-sufficient.


Individual Therapy and Education:

Therapist will provide services that are matched to an individual’s unique needs and environment. Individual family members learn to problem-solve by developing an understanding of their behavior which allows them to modify their behavior in a positive and appropriate manner.


Individual Substance Abuse Counseling and Education:

Therapist will provide services which focus on the individual’s substance abuse issues. Therapist will provide drug education as well as assist the client in understanding his or her triggers to drug use. Therapist will complete a chemical health assessment with each client so that a history of substance abuse can be obtained. The Therapist will assess the client’s need for inpatient substance abuse treatment services and will provide appropriate referrals if deemed necessary. Therapist will assist all clients in developing a relapse prevention plan.


Group Therapy:

Group sessions are 14 weeks for youth who need continued skill development. Components are: anger management, assaultive behavior, self-advocacy, decision making, substance abuse, etc.


Behavior Health:

Case Managers provide para-professional counseling, identify problems, and address them proactively. Teach independent living skills, socialization, and enhance community integration. Youth participate in group, clinical services and are provided 24 hour crisis intervention.


Shelter Care and Supervision Services:

New Hope and Hope Haven’s Facilities provide shelter care and supervision that are appropriate for children requiring emergency placement and short term care while working towards permanency of placement. Facilities specialize in working with dual ward and hard-to-place youth.


Domestic Violence Transitional Homes:

CI, Inc. provides transitional living to women and their children who are victims of domestic violence. Services include counseling, assistance in vocational or education enrollment, life skills, advocacy services, parenting skills training, job readiness and support in securing assistance in any other area required.


Parenting Skills Training:

The course objectives include, but not limited to, human and child growth and development, effective parent-child communication, appropriate disciplinary techniques, etc.


Medication Monitoring, Life Skills, and Personal Care:

Therapists and/or Case Managers are responsible for providing medication monitoring, support and training to SMI adults. Individual treatment plan to meet unique consumer's needs is developed and implemented. Therapists and/or Case Managers also teach life skills and develop support system for consumer.


In Home Services (Family Builders):

The In-Home Teams will use the family’s cultural strengths and informal support networks in empowering them in service delivery and accomplishing the goals outlined in the Assessment and Family Plan as well as making recommendations.

Services may include but not limited to: crisis intervention counseling, substance abuse education and counseling, individual, family and marital counseling to include trauma and abuse, domestic violence education and counseling, parenting education and training, job readiness training, conflict resolution skills and anger management, problem solving skills and stress management, home management and nutrition, communication and negotiation skills, behavioral management and modification, development of linkages with community resources and facilitation of family meeting.
